I, the undersigned parent/guardian, do give permission for my child named above to attend the First Baptist Church’s Preschool VBS. In the event that my child would sustain injury or illness during the period of VBS, I will be contacted as soon as possible. Failure to reach me shall not prevent an application of immediate necessary medical treatment; therefore, I hereby authorize the VBS staff to obtain or provide medical treatment for such injury or illness during the VBS, and I hereby hold the VBS staff and sponsoring organizations, as well as it’s representative, harmless in the exercise of this authority. I further acknowledge and understand that I will be responsible for any medical bills that may be incurred on behalf of my children for treatment of such injuries or illnesses. I further release First Baptist Church, the sponsoring organizations, and it’s representatives from any claims for personal illness or injury that my child may sustain during VBS. I further acknowledge and understand that my child will be responsible for his/her failure to abide by the rules and regulations of the VBS.

I, the undersigned parent/guardian, do give permission for photos and videos that may include my children to be used in ways First Baptist Church chooses which may include brochures, posters, publicity, newspaper, internet website, etc.
