I, the undersigned parent/guardian, do give permission for my children named on the reverse to attend First Baptist Church’s Sports Camp. In the event that my children sustain injury or illness during the period of camp, I will be contacted as soon as possible. Failure to reach me shall not prevent an application of immediate necessary medical treatment; therefore, I hereby authorize the camp staff to obtain or provide medical treatment for such injury or illness during the camp, and I hereby hold the camp staff and sponsoring organizations, as well as it’s representative, harmless in the exercise of this authority. I further acknowledge and understand that I will be responsible for any medical bills that may be incurred on behalf of my children for treatment of such injuries or illnesses. I further release First Baptist Church, the sponsoring organizations, and its representatives from any claims of personal illness or injury that my children may sustain during camp. I further acknowledge and understand that my children will be responsible for his/her failure to abide by the rules and regulations of the camp.
